Dr. Chandria Johnson regretfully announces that her practice is closed due to current economic environment in the Milton area. Our cost for malpractice insurance, office equipment and supplies along with very low patient volumes made it financially impossible to keep our office open.  Dr. Johnson has moved to practice medicine in another state.

We would like to thank all of our patients, staff and community for their support during our time in Milton.  We loved the area and community; however there was not the amount of patients to support our practice without Dr. Johnson having to open other offices in nearby communities and delivering at more than one hospital to make ends meet.  She felt that the risk of trying to cover more than one hospital and the time commuting was not worth the risk to patient safety and her time away from her family.  We hope the best for our patients and the little lives we were part of welcoming to this world.

Requests for medical records will be handled in a timely fashion. If you wish to request your medical records, please click the link below to download the release of records form. Complete the form and either fax or mail to our records office. You may choose to have your records sent to yourself or to a physician’s office. Please indicate your preference on the form.
